This is the permission slip for all events that occur on-site at the 4 parishes that are a part of The GROVE Youth Ministry community. Once submitted this Permission Slip is valid from August 1, 2021, through July 31, 2022.
This form grants The GROVE Youth Ministry permission to take and use photos during the various events that may be used to promote the ministry on various social media platforms without identifying the teens in the photos or videos.
